Inventory of Transition Protocols Facilitates Collaboration

Puzzle - 4 pieceAt a recent meeting with my colleagues in the Southeast region, I learned about a transition protocol resource developed by our counterparts in KFL&A.

It is called a Protocol Primer and a 2015 e-version is available HERE.

First developed in 2014 by the KFL&A Disability Services Committee, the primer contains protocols and service agreements that support successful transitions during the early years and school years together with transition to adult services.  Also included are tools and resources that help parents and caregivers navigate the service system.

Members of the CYSN reading this blog may feel that a similar resource would enhance communication and collaboration, and further support the philosophy and practice of No Wrong Door in Hastings-Prince Edward by service providers delivering programs to children and youth with disabilities, mental health issues and special needs.
